How to apply UDID card and Disability Certificate?

Activities avoided, experiences excluded, food forbidden, milestones unmet, and many such challenges and obstacles are faced by the special need kids, not out of their choice though. These hindrances can hit some families hard and will look at these as a tragic destination, while some would want to seek help and guidance that is needed to convert the weakness into amazing strengths.
Though every special need kid is unique and every family is different, there are some common links that are sought upon especially in terms of care, academics, jobs, and independent bright future. Here are some guidelines to facilitate these -

It can be issued by a government hospital after filling up a form and getting checked by a board of doctors and therapists. Delhi government has distributed the area zone wise. One should apply in the hospital which falls in your zone.
